The Center for Physical Excellence is located in Prescott Valley, Arizona and serves the greater Central Northern area of Arizona and Yavapai County.

The wellness center, or Athletic Performance Center is located at the Main Prescott facility and is offered to patients as an integral part of their ongoing therapy as well as the public as a well-equipped fitness facility.
